POPULATION-BASED PRIMARY CARE

The health of the community depends on several processes and people and on individuals working together in order to take care of one another. Environment, income, culture, trust, language, tradition, cooperation and religion and services such as police, fire protection, education, medical care and sanitation   and all coordinate to make the place and its people relatively fit and healthy.

Due to the unorganized health care system in most of the places it is sometimes becomes difficult for the communities to organize the medical services which affect impact people’s health.  More complicated however is the work of directing our medical or health care which they aim on maintaining both the community and health of persons as a whole. Population-based main care is a method to offer health services therefore the health of both the community and individuals as a whole is improved and maintained. The population-based main care practice is a clinical practice which aims to offer ninety percent of health care services to the people in a way that interests ninety percent of the individuals in the community in using that medical practice. Population-based main care enables a clinical practice in order to work together with other community business and organizations, such as schools, town government, health clubs, fire and rescue and police departments, at the same time as it ensures all town people get all the health care services they require.

Population based primary care is not practiced in all parts of the US; however it is practiced in other parts of the world, where the cost is lower and people of the country are healthier. Little Scituate, Rhode Island  is the only place in the united states of America which aims to provide population-based primary care  to all the people in living in Scituate and  wants to be the healthiest place in the United States  as a result, a community of people who are healthy together.
