Introduction

Major role of nurse is to take care of patient. Patient care by nurse goes systematically starting as mentioned below.

Patient care is considered as significant, though caring is carried out as a holistic framework. Florence Nightingale was the first nurse researcher.

Clinical setting/triage

Nursing care is essential when patients come with symptoms like flu and they have to be sent to specially allocated area. Patient care is nurse’s responsibility. Nurses are in charge for observing the chance of infection spread. They must carry out aseptic techniques and surgical scrub techniques in a safer way.

Initial assessment

Nurses take care of patients by initial diagnosis. To start with, nurse will check respiration rate for a minute to ensure whether a patient is getting saturated oxygen. If the nurse found it is high or less, they report to their senior staff for superfluous patient caring.  They also record the details such as date of onset of infection and their symptoms. According to symptoms, they are subjected to diagnosis like blood test, scanning, chemotherapy, tomography and surgery. Nurses are instructed to be clad in all personal protective equipments like mask, eye glass, gloves and aprons whenever they come in contact with patient. After which they report the diagnosis results to senior medical officer.

Nurse care plan

Assessment results direct nurse to implement the actions to reduce the patient sufferings. This planning is middle part of patient care. They focus mainly on existing nursing problem. Initially, they collect related factors responsible for nursing problem. They adopt PES system that is Problem, Etiology and Symptoms.

Implement

Nursing care is planned in such a way that patient is in good health after nursing interventions. Nursing interventions are so specific. Patient must understand need and time for implementing nursing interventions that are drafted by nurse.

Evaluation

They are carried out by setting date for purpose. This shows whether nurse care plan is effectively carried out or missed somewhere in the middle and reason for the same. Continuous follow up activity of patient health improves the health quality of an individual.
